No principled basis for shared effect-modifier treatment classes

The shared effect modifier assumption is defined relative to a set of treatments assumed to carry identical interaction vectors, and it is the restriction that makes most small-network multilevel network meta-regression and most target-transportable MAIC or STC identifiable at all. Nothing governs how that set is chosen. In practice it is enlarged until the model is estimable: one metastatic prostate cancer analysis grouped every treatment except the reference into a single class spanning docetaxel chemotherapy, androgen deprivation, a non-steroidal antiandrogen and three androgen-receptor pathway inhibitors, across twelve studies with individual patient data from one of them (Shore et al. 2026, doi:10.57264/cer-2025-0100); a relapsed refractory myeloma application shares interactions across a CAR-T product, a nuclear export inhibitor and an antibody-drug conjugate (Maciel et al. 2024, doi:10.1016/j.jval.2024.04.017); and the psoriasis application that validated the method used mechanism-defined classes (doi:10.1177/0272989X221117162). Widening the class strictly weakens the identifying assumption while strictly improving estimability, so the incentive runs toward the widest partition a reviewer will accept, and the partition is typically settled after the analyst has seen which version converges. What does not exist is a criterion linking class membership to an observable implication, a measure of how much a target contrast moves across admissible partitions, or a requirement to report the partition and its justification as an identifying assumption rather than as a modeling detail. The relaxations software provides operate within a fixed partition rather than over it: multinma’s class_interactions argument moves between common, exchangeable and independent interactions for a given class structure, and the class-effect model selection algorithm of Perren et al. (doi:10.1177/0272989X251389887) selects among class-effect structures on the treatment effects, not among partitions of the interaction assumption.

ImportantA study has been run against this problem

No. When an aggregate-only treatment’s interaction differed from both candidate classes’ (by 0.3) while theirs agreed, every admissible partition returned the same answer biased by -0.30; the range over partitions was narrow (0.14), contained the truth in 5% of replicates, and the only available fit test rejected at its nominal 5%. The widest partition covered 0.37. The range widened and contained the correct partition only when the candidate classes disagreed with each other. Read the study below.

Statement

Why it is open

The assumption restricts interaction vectors for treatments whose interactions are, by hypothesis, not separately estimable, so it is only testable in networks that do not need it, and any diagnostic is weakest exactly where the partition carries the most identifying weight. Class membership is a scientific claim about shared mechanism of effect modification, which is not the same as shared mechanism of action and is rarely the subject of any direct evidence, so there is often nothing external to appeal to. Widening the class is not visibly wrong: a wider class fits, converges and returns narrower intervals, so the diagnostic signature of an indefensible partition looks like success. A partition-sensitivity analysis is easy to describe and nobody has built it, partly because the number of possible partitions grows quickly and there is no agreed way to restrict the candidate set to clinically defensible ones. Reporting practice hides the choice, since the partition is usually stated in a single clause of a methods section, if at all, and does not appear among the items that appraisal checklists ask for.

Work What it contributes
Phillippo, Dias, Ades, Belger et al. 2020, Multilevel Network Meta-Regression for Population-Adjusted Treatment Comparisons, JRSS-A 183:1189-1210 Defines the model in which the shared effect modifier assumption becomes the identifying restriction when aggregate-data arms cannot support treatment-specific interactions; the assumption is stated relative to a treatment set whose construction is left to the analyst.
Phillippo, Dias, Ades, Welton et al. 2023, Validating the Assumptions of Population Adjustment: Application of Multilevel Network Meta-regression to a Network of Treatments for Plaque Psoriasis, Medical Decision Making Uses mechanism-defined classes and relaxes the shared interaction one covariate at a time, comparing posteriors and model fit; the relaxation is over covariates within a fixed partition rather than over the partition itself, and its power is conceded to be low.
multinma R package, nma() argument class_interactions Implements common, exchangeable and independent interaction structures for a class of treatments, so the strength of the sharing can be varied; the classes themselves are supplied by the user and no diagnostic reports sensitivity to that input.
Perren, Pedder, Welton and Phillippo 2026, Network Meta-Analysis with Class Effects: A Practical Guide and Model Selection Algorithm, Medical Decision Making 46:275-295 Gives a model selection algorithm for class-effect structures and sharing of within-class variability across sparse classes; it applies to class effects on the treatment effects, and its adaptation to partitions of the interaction assumption has not been made.
Shore, Morgans, Paracha, Thom et al. 2026, Population-adjusted network meta-analyses provide new insights into the efficacy of treatment alternatives for metastatic castration-sensitive prostate cancer, Journal of Comparative Effectiveness Research 15 Groups all treatments except the reference into one class with identical effect-modifier impact, over twelve studies with individual patient data from one trial; the partition is stated in a sentence and is not accompanied by a justification or a sensitivity analysis.
Maciel, Jansen, Klijn and Towle 2024, Implementing Multilevel Network Meta-Regression for Time-To-Event Outcomes: A Case Study in Relapsed Refractory Multiple Myeloma, Value in Health 27:1012-1020 A second application that shares effect-modifier interactions across mechanistically dissimilar agents in order to estimate the model, showing that the widest-class strategy is not confined to one therapeutic area.
NICE DSU Technical Support Document 18: Methods for Population-Adjusted Indirect Comparisons in Submissions to NICE Sets out what a population-adjusted submission must justify; the treatment set over which effect modification is assumed shared is not among the items it requires to be reported or defended.

Probable solution or research direction

Build the partition-sensitivity diagnostic first, since it is the cheapest change that would alter practice: refit under every clinically defensible partition and report the induced range of the target contrast next to the point estimate, so a contrast whose sign depends on the class structure becomes visible. Define the admissible candidate set with clinicians before fitting and record it in the protocol, so the partition is a prespecified identifying assumption rather than a consequence of which model converged. Adapt the existing class-effect model selection algorithm from treatment effects to interaction vectors so partitions can be compared on fit where the network supports it, and state the network size at which that comparison has power rather than assuming it does. Where the partition cannot be checked, replace the equality constraint with a hierarchical prior over interaction vectors around a class-level mean and report how the contrast moves as the shrinkage is loosened, which multinma supports within a partition and would need extending across partitions. Finally, require the partition and its justification in reporting, on the same footing as the covariate list.

Our study

A partition-sensitivity display for shared effect modifiers is stable exactly when every candidate partition is wrong

The question put to it. Does the choice of which treatments share an effect-modifier interaction move a target contrast, does the widest estimable partition fail silently, and does the range over defensible partitions contain the truth?

What it found.

Results.

  • Widest partition bias -0.30 and coverage 0.37 at the primary cell, with the fit test rejecting in 5% of analyses.
  • The fit test detects disagreement between treatments with identified interactions (41% when C differs from B) and never the aggregate-only treatment’s own departure.
  • With the candidate classes disagreeing, {B}{CD} or {BD}{C} was unbiased when D truly shared that class’s interaction.

Verification

Verdict. Confirmed open. One reader proposed this from a single applied analysis, with two further published applications cited as corroboration, and the reviewer accepted the treatment partition as an identifying modeling choice distinct from covariate selection and from the power to test a fixed shared-interaction restriction. No methodological work addresses partition choice; the evidence that it is unconstrained is that published applications range from one class containing everything to mechanism-defined classes with no stated criterion in any of them.
